Forms Needed for Classic & Elite Players Playing in VSLI:

• VYSA Medical Release Form - needed for all players on Classic teams and Elite teams playing in the VSLI.  Form should be notarized
• VYSA International Clearance Waiver - needed for any player who has ever lived outside the US
