2 - 1” square ID style photos – SEE PHOTO NOTES BELOW - PLAYER’S NAME AND TEAM NAME MUST BE ON THE BACK
Completed and notarized medical release form (US CLUB version)
COPY of birth certificate – new CUFC players only
Signed, Player & Parent Code of Conduct
VSLI TEAMS:
(U12 Elite, All U13 – U19 teams)
2 - 1” square ID style photos – SEE PHOTO NOTES BELOW - PLAYER’S NAME AND TEAM NAME MUST BE ON THE BACK
Completed and notarized medical release form (VYSA version)
ORIGINAL proof of birth, new CUFC players and players moving from an ADSL team to a VSLI team
Self addressed, stamped envelope, for return of proof of birth
Signed, Player & Parent Code of Conduct
International Clearance form if you have lived outside the USA
PLAYERS MUST SIGN PLAYER PASS
